"This is a consultation for patient John Smith on May 12, 2025. The patient presented with persistent abdominal pain for the past three weeks, primarily in the lower right quadrant, accompanied by nausea and occasional fever. The pain started gradually and has worsened over time. He denies any recent trauma or injury. The patient has a history of hypertension, managed with medication, and a previous appendectomy ten years ago. He is allergic to penicillin. On examination, there is tenderness in the right lower quadrant with rebound tenderness. Bowel sounds are normal, and there are no palpable masses. My preoperative diagnosis is acute appendicitis, with a differential diagnosis of possible diverticulitis or mesenteric adenitis. We discussed the need for an urgent appendectomy, including risks such as infection or bleeding, and benefits of resolving the condition. The patient consented to the procedure. The plan includes preoperative blood work, imaging, and antibiotics. Postoperative care will involve pain management and monitoring for complications. The patient asked about recovery time, and I explained it typically takes two to four weeks. Signed, Dr. Emily Carter."
